Auditor General Tables Several Reports Including Two Investigation Reports, the 2024 Report on the Fraud and Waste Hotline, and her 2024 Annual Report

Ottawa Today, Auditor General Nathalie Gougeon tabled several reports including two investigation reports, both stemming from reports made to the Fraud and Waste Hotline. 

The first investigation, an Investigation of the Trillium Line Training Program identified a lack of evidence to demonstrate that specific City diesel rail instructors were sufficiently qualified and fully trained before they commenced instruction to the diesel rail operators. The investigation further highlighted that in-class testing administered by the vendor did not consistently maintain the expected level of integrity, as instances of inappropriate behaviour were observed. The investigation emphasized the need to demonstrate how staff met the required training plans and experience requirements. The investigation report included five recommendations, all of which were agreed to by management. 

The second report tabled by Deputy Auditor General Joanne Gorenstein was a Building Code Services Investigation. This investigation assessed allegations concerning the issuance of building and occupancy permits under the Ontario Building Code. The investigation emphasized the need to enhance processes and documentation to demonstrate due diligence was performed when issuing building and occupancy permits. The report included five recommendations, all of which were agreed to by management. 

Mme. Gougeon also tabled her 2024 Report on the Fraud and Waste Hotline which received a total of 328 cases in 2024, representing a 16% increase from the prior year.  The report speaks to various investigation conclusions and the corrective actions taken by management.  

Finally, Mme. Gougeon tabled her 2024 Annual Report, revisions to the 2024-2025 Audit Work Plan and a three-year strategic plan for the team.  Most notably, the Auditor General has added the Audit of Councillors Office Expenses to her current plan.
